The stars of the reality show –“Keeping up with the Kardashians” have contributed their top knot chignon as a major fashion statement to the rest of the world, but the reality of it is much harsher.
Frequently wearing the hair in a particularly tight ponytails, pigtails, braids or buns, results in a form of gradual hairloss or alopecia called Tractional Alopecia.
It is also seen in women who blow dry or use the hair iron excessively, using hair extensions, hair relaxers and rollers over a prolonged period of time. The traction used to achieve the desired looks also leads to this form of alopecia.
Tightly worn compressive safety helmets, helmets used for motorcycles, skiing, cycling and snowboarding, constantly rub localized areas of the scalp and hair, resulting in alopecia.
This also implies that the main cause for this condition is mechanical instead of a physiological or anatomical imbalance.
The patient complains of itching, redness, short broken hairs, thinning of the hair, hair loss and sometimes even complaints of folliculitis – which is inflammation of the hair follicles.
On physical examination there is recession of the hairline because of the constant traction. It is mostly seen in the frontal region, but can be seen anywhere on the scalp based on the traction. “Fringe sign” is positive – which means that some hair is retained along the frontal and/or temporal rim of the hairline.
Treatment of traction alopecia mainly constitutes avoiding the triggering factors. So wearing the hair loose, avoiding the use of harsh chemicals and relaxers on the hair and also avoiding use of the hairdryers or hair irons.
Medical treatment includes treating any infections with oral or topical antibiotics, topical antifungals or steroids wherever necessary. Hair growth supplements which have Biotin and topical Minoxidil solutions also provide good response. In worse case scenarios, even hair replacement surgeries can be recommended.
